"Guaranteed acceptance" sounds absolute, but in practice it splits into two quite different products. Strict guaranteed issue means the carrier issues the policy with no health questions at all — and only a small number of state-specific programs offer that. They charge significantly higher premiums and almost always include a 2–3 year graded benefit: if the insured passes away in years 1–2, the beneficiary receives premiums paid plus interest, not the full death benefit.
For most applicants, the practical equivalent is simplified-issue underwriting. The carrier replaces the medical exam with a short health questionnaire — five to ten yes/no questions on major conditions, recent hospitalizations, and current medications. Your answers plus your age and tobacco status place you in a Preferred or Modified tier, and your premium is calculated from there.
